Fee Schedule

Initial Session, 45 minutes, with diagnostic assessment                                 $150.00

Individual Session, 45 minutes                                                                             $120.00

Missed Session                                                                                                           $60.00

Late Cancellation (less that 24 hours)                                                                   $60.00

I will determine your eligibility and coverage limits, and bill your insurance.  I will accept the rate they pay, plus whatever co-pay, co-insurance or deductible your insurance requires.  If I am calculating your deductible or co-insurance I will use the fees listed above.

Payment is due at the time of the session.
